Jeff Lyth has been a health and safety professional in BC for 34 years. He was profoundly impacted in 2012 by several high-profile workplace accidents in BC. Disappointed by the analyses coming from conventional occupational safety thinking, he began searching for better answers and soon found them in the work done by Erik Hollnagel, Sidney Dekker, and others, which has become known as human and organizational performance.
His current work involves helping organizations to understand these recent developments and to apply practical tools in their organizations. He also facilitates a variety of courses and workshops and assists legal teams in serious and fatal accident investigations.
